Headcount Meal Counts
For Use by Emergency Shelters and Afterschool Programs and Some Child Care Centers/Adult Daycare Program
This form is for programs that have a set menu on file with FEEDING OUR FUTURE. Fill in the attendance and total participants eating snack/supper. Child care programs that offer two meals and one snack or two snacks and one meal are able to use this form.

Weekly Meal Counts
For Use by Child Care Centers and Adult Daycare Programs
This form is for child care centers and adult daycare programs that offer more than two meals and one snack or two snacks and one meal. Add the dates and days of the week before taking attendance and meal counts.
CACFP Annual Form
Child Care Center
Annual forms are used to determine if for profit centers are eligible to participate in the CACFP. Additionally, the annual forms are used to maximize your reimbursement. Please add the CACFP annual form to your enrollment packet. Forms are valid for one year from the date signed.
Meal Pattern Requirements
Meals and Snacks Must Meet the Meal Pattern Requirements
Programs must serve meals and snacks that meet the meal pattern requirements for the age of the participant. FEEDING OUR FUTURE reviews all menus and receipts to ensure the meal pattern is being met. Any meal or snack that does not meet the meal pattern requirements will be disallowed. Portion sizes listed are the minimum serving size allowed.
